•Thank you for your interest in the eating behavior survey!  This survey takes about 30 minutes to finish.  Virginia Quick, RD, from Rutgers University, along with Stefano Gunadalini, MD, from the University of Chicago, have undertaken a research project investigating the eating behavior of young adults.  Your participation in completing this survey for our research study will help us understand eating behaviors in young adults.  You will be asked questions regarding your demographics (e.g., age, gender, ethnicity, religion, height, weight, and health status); eating practices, and questions that assess psychological, body image, family and social environment, and intrapersonal factors.  Information on your name, email address, and phone numbers will be collected as part of the study so that we may contact you if you win one of ten $25 America Express gift cards.  You may receive no direct benefit from participating in this study.  We are hoping that approximately 2,500 young adults will participate in this study in total, with 400 from the University of Chicago.
•Participation is voluntary, free of charge, and poses minimal risk.  You may stop participating at any time and/or choose to not answer questions with which you are not comfortable.  If you choose not to participate, there will be no penalty or loss of benefits to which you are otherwise entitled to.  All information you provide will be kept confidential and stored in a secure location.  Only the research team, made up of Dr. Guandalini, Ginger Quick, RD, and Carol Byrd-Bredbenner, PhD, RD, along with the Institutional Review Boards at Rutgers University and the University of Chicago will be permitted to see the information you provide, except as may be required by law.  At the end of the study, we will destroy all records associated with it.  Reports about this project will not include any identifying information about you.  All study data will be kept secure until the study ends in 5 years.  This study involves the potential risk of a loss of confidentiality in the case that our files are stolen, but we will take precautions to ensure that your data is kept confidential. All of the participant’s personal information will be kept separate from the survey data by placing identity codes for each participant. These identity codes and survey responses will be saved in files on a secured computer.  Ginger Quick, RD, will be the only one with access to them via a secured username and password.
•Those who complete the survey will be entered into a raffle to win 1 of 10 $25 prizes, in the form of an American Express gift card.
